Parade Registration Form

RELEASE, INDEMNIFICATION, AND HOLD HARMLESS

In consideration of the Town of Cicero, Hamilton County, Indiana, permitting the undersigned to participate in the town’s 4th of July Parade, the undersigned does hereby release and discharge, and agrees to indemnify and hold harmless the Town of Cicero, Hamilton County, Indiana, and Lights Over Morse Lake, Inc., its respective employees, agents, and assigns of and from all manner of actions, suits, liabilities, claims, causes of action, and demands of whatever kind and whatsoever, in law or equity, for any loss or damage to any person, property, or organization hereof because of, or on account of, the actions or omissions of the undersigned or its employees, agents, and assigns, and for any loss or damage to the undersigned’s person and/or property which may occur in connection with the event known as the Lights Over Morse Lake 4th of July Parade.

Important Information (PLEASE READ)

  1. All parade positions will be determined prior to Monday, July 4th.
  2. If you have a special request, please write it in the comments section of page 1 of this entry.
  3. If you miss the entry mail in deadline, please bring your completed form to the lineup area with you.
  4. No entries will be allowed to participate in the parade without a signed entry form.
